Understanding the results of your thyroid blood test often feels like trying to read a foreign language. Your doctor hands you a piece of paper, points to a number, and says, “You’re fine.” Yet, as you leave the clinic, you still feel drained, plagued by brain fog, and sensitive to temperature changes. This disconnect between your doctor’s claims and your lived reality can leave you questioning your own sanity.

Let’s explore what the numbers on your thyroid blood test results really mean, and, more importantly, the things your doctor might not be telling you.

Understanding Thyroid Blood Test Results: The Basics

Understanding thyroid blood test results involves paying close attention to a few key markers:

  • TSH (Thyroid-Stimulating Hormone): This is the pituitary gland’s way of yelling at your thyroid to encourage T4 production. High TSH levels typically mean your thyroid isn’t producing enough thyroid hormone (hypothyroidism). It’s important to note that–at times–underproduction of thyroid hormone is a normal, protective response as the body slams the brakes to prevent hyperthyroidism from exposure to iodine excess. Conversely, low TSH levels usually indicate that your thyroid gland is overproducing thyroid hormone (hyperthyroidism) or, sometimes, that your pituitary gland is suppressed and is not producing enough TSH (central hypothyroidism).
  • Free thyroxine (T4): The inactive form of thyroid hormone. Your body converts it into T3, which is essential for various bodily functions like metabolism, temperature regulation, and energy production.
  • Free triiodothyronine (T3): The active form that actually does the work in your cells.
  • Thyroid antibodies: These show up when your immune system attacks the thyroid gland, leading to disorders such as Hashimoto’s or Graves’ disease.

Normal ranges vary by lab, but TSH is generally considered normal between 0.4 or 0.5 and 4.0 or 5.0 mIU/L.

Simple enough, right? Except it’s not.

Interpreting Thyroid Blood Test Results: Why “Normal” Doesn’t Always Mean Healthy or Feeling Yourself Again

Here’s where things get interesting—and often frustrating.

Interpreting thyroid blood test results the way most US doctors do means looking almost exclusively at TSH. They declare you’re “fine” if your TSH falls within the normal range, blame your symptoms on low-hanging fruit like anxiety, stress, weight, their assumption of your lack of exercise even if you’ve just come back from hiking the Appalachian Trail, and send you on your way.

However, the research is increasingly detailed: TSH alone is not an adequate measure of thyroid function.

A 2025 paper in the Journal of Clinical Endocrinology & Metabolism called the current TSH-dominant paradigm “inadequate” and said it has “distorted medical practice and research for 50 years.” The authors argued that hypothyroidism “must be defined physiologically as insufficient T3 effect in some or all tissues.” They also stressed that the most effective approach for diagnosing thyroid dysfunction is free T4 (FT4) and free T3 (FT3), considered together.

Let me repeat that: A single TSH number cannot tell you whether your tissues are actually getting the thyroid hormone they need.

So, if you still feel bad despite a doctor telling you your labs are normal, there is a very good chance that your doctor is evaluating you with an incomplete, outdated framework.

Understanding the Meaning of Thyroid Blood Test Results: What Your Doctor Isn’t Telling You

The meaning of your thyroid blood test results goes beyond whether your TSH is in range. Other information a full blood panel provides insights into include:

1. T4 to T3 Conversion

Synthetic T4, like levothyroxine and Synthroid, is the standard treatment for hypothyroidism in the US. Each person’s ability to convert T4 to T3 is unique. Some patients have genetic variations, nutritional deficiencies, or other factors that can impair hormone conversion. Consequently, their cells can be deprived of active T3 even when their TSH and T4 levels appear completely “normal” in lab results.

There’s mounting evidence that suggests synthetic T4 alone may not be enough to deliver adequate T3 to all tissues. Yet most doctors never test free T3.

Some individuals have reported anecdotally that they feel better taking natural desiccated thyroid (NDT) derived from cows (bovine) or pigs (porcine) instead of levothyroxine. It should be noted, however, that certain endocrinologists steer clear of desiccated thyroid sourced from cows and pigs. This avoidance stems from the fact that bovine and porcine desiccated thyroid extracts have a fixed T4 to T3 ratio of roughly 4:1, a stark contrast to the human thyroid gland, which naturally releases these hormones at a ratio of approximately 13:1 to 14:1.

Still, some patients report doing better taking NDT under medical guidance, although this may be just part of the puzzle for what they need. An accumulation of tiny changes may be needed to improve the patient’s health with careful assessments, adjustments, and regular monitoring by medical teams.

2. Iron Status

Iron deficiency can impair thyroid metabolism. Studies have shown that women with anemia and thyroid disorders can improve their thyroid hormone levels with iron supplementation. The problem is that many doctors evaluating thyroid function don’t even bother checking ferritin and serum iron levels. Importantly, many clinicians in certain countries encourage a food-first approach.

In the anemia subreddit, I have observed numerous patient-reported side effects resulting from supplements and iron IV infusions. Interestingly, in that subreddit, many German patients reported being told by their physicians to eat classic German foods like liver and blood sausage twice a week if anemic instead of an automatic prescription for a pill.

In another case, a homebound patient highly suspected that her constant dry mouth from iron supplementation was responsible for consistent dental problems and the need for multiple costly root canals, where transportation to a medical center and accommodation of her as a bedbound patient on a stretcher created immense access problems. This does not mean you should completely avoid supplementation; rather, it is important to recognize that supplements carry potential risks and to advocate for a comprehensive risk-versus-reward assessment from your healthcare providers during care planning.

3. Folate and Methylation

Patients with hypothyroidism tend to have significantly higher homocysteine levels than healthy people. Folate and B12 are critical for homocysteine metabolism, which in turn affects thyroid function through methylation pathways. Your thyroid may not function optimally, even if your TSH looks fine, if you’re folate deficient.

4. Absorption Issues

Iron deficiency, celiac disease, and other gastrointestinal issues can impair the absorption of thyroid medication. This means you can still have inadequate thyroid hormone levels even if you’re getting the right dosage of synthetic T4 because your gut isn’t properly absorbing it.

What Does a Thyroid Blood Test Show? Less Than You Think

So what does a thyroid blood test show? It shows a snapshot of a few hormone levels in your blood at a single point in time.

What it doesn’t show is:

  • How well your tissues are using those hormones
  • How effectively T4 is being converted into T3
  • How existing nutritional deficiencies and absorption issues impact thyroid function
  • Whether your symptoms are related to thyroid hormone resistance at the cellular level
  • How energetic you feel (a “normal” T4 doesn’t mean you feel fine)

That’s a lot of missing information. And the patient narrative adds more to the numbers. Has the patient tried natural desiccated thyroid (NDT) and done a bit better on that than levothyroxine? Sure, that’s subjective data, but it can also be potentially helpful information.

How to Read Thyroid Blood Test Results Like a Pro

Learning how to read thyroid blood test results means looking beyond the TSH and asking better questions:

  1. What’s my free T3? If your doctor didn’t order it, ask why.
  2. What’s my free T4? If it’s at the bottom of the range, that might be a problem even if TSH is “normal.”
  3. What’s going on with my thyroid antibodies? Hashimoto’s is the most common cause of hypothyroidism in the US, and many doctors never test for it.
  4. What are my current ferritin and serum iron levels? Iron helps regulate thyroid function.
  5. How do my folate, vitamin B12, and iron levels look? The first two impact homocysteine levels and methylation pathways, and the third is converted into T3 and T4.
  6. 6. At what level am I likely to feel my best? Some patients feel better with TSH around 1.0 mIU/L; others need it closer to 2.5 mIU/L. The “normal” range is broad, and your optimal level might be different from someone else’s. Carefully examine any past lab reports if they are available to you. You might have had TSH assessments thrown in by your gynecologist during annual exams in the past when you were feeling fine and see that your baseline tended to be about 1.5 mIU/L, but now you’re hovering at 4.5 mIU/L and told you’re fine even though you’re exhausted all the time. These past labs can give you proof to ask your physician whether shooting for a TSH of 1.5 (while being careful not to overshoot into hyperthyroidism) might be more appropriate for your body. Perhaps with an audit of your dietary iodine intake, you might find that you were not eating the recommended daily allowance and that your body needed additional daily dietary iodine intake slowly re-achieved.
  7. What other factors can affect thyroid health? There are many lifestyle factors that can affect thyroid health as well.

Learning how to read thyroid blood test results isn’t just about knowing the normal ranges for thyroid hormones; knowing what gaps to look for and which questions to ask your doctor is equally as important.

The US Healthcare System’s Dirty Little Secret

Here’s the uncomfortable truth: The US healthcare system is not set up to investigate complex, nuanced thyroid issues.

Insurance reimbursements are structured around 15-minute appointments. Labs are processed in bulk. Guidelines are written for populations, not individuals. And the TSH-centric model is so deeply entrenched that many doctors simply don’t know, or don’t have time to care, about the limitations of a TSH-focused approach for diagnosing thyroid dysfunction.

Patients who have had radioactive iodine ablation therapy for Graves’ disease or thyroid cancer are often told their TSH and free T4 levels are “within range” and sent home while they continue to struggle with debilitating symptoms for years. The research on persistent symptoms after RAI is abundant, but it rarely translates into better care.

Some US citizens have found that seeking care from endocrinologists in Colombia, or other South American countries with a more comprehensive approach to addressing thyroid dysfunction, gives them something the US system rarely offers: a clinician willing to investigate further, assess the full picture, and actually spend time educating their patients.
